If you become unemployed and have worked in Vermont anytime in the past 18 months, you may be eligible to receive unemployment insurance. Once you become totally or partially unemployed, the time to establish a new claim is during the first week you work less than 35 hours. Required Information
Following are the details you will need while filing your claim for the first time. Keep everything ready so that the filing process will not suffer any unwanted delays.
- SSN – Social Security Number
- Contact addresses – email and residential
- Your telephone number
- Alien registration number if you are not a citizen of the U.S.
- Details of severance pay, if you received any
- Possible date of returning to work, if you are on a temporary layoff
- State issued ID number of Driver’s License number
- Details of your bank account for depositing the benefit check
- If you are ex-military, your Military Form DD-214
- If you are an ex-federal employee, your form SF-8
You also need the following details about every employer you worked for in the past 18 months.
- Name, address and contact numbers of every employer (including the zip code)
- Payroll addresses
- Telephone number(s) of the employers you worked for
- Joining and relieving dates of jobs served in the last 18 months
- Reason behind job separation
